Officer Praise/Complaint Form
Per ARTICLE XXXV of the POLICE OFFICER BILL OF RIGHTS:
9. The Police Chief or his designee may investigate any police policy or procedure violation by a department member, regardless of the source or format of the initial information. All unsigned complaints will be sent to the sergeants (or acting OIC) for investigation. The sergeants can send the complaint to the chief for further investigation by a majority vote or vote to dismiss the claim as unfounded. Sergeants may investigate signed complaints at the request of the chief of police. If any sergeant is associated with the complaint, then said sergeant(s) shall recuse themselves from the panel. In the event of a tie, the Chief shall act as the tiebreaker. Complainants shall be encouraged to complete and sign a standardized complaint form, informing the complainant that providing false statements or accusations is subject to penalties under 4904, relating to Unsworn Falsification to Authorities.
